In consideration of compensation as indicated below, receipt of which is hereby acknowledged, I _________________________ (hereafter "Model") do hereby irrevocably authorize the Photographer and/or representative ___________________________(hereafter "Photographer"), Photographer's legal representatives, assigns, and those acting under Photographer's permission and on Photographer's authority to copyright, publish, and use in conjunction with self-promotion in all forms and media and manners, any depictions of myself made by Photographer or Photographer's representative, in which I may be included in whole or in part, alone or in conjunction with other persons, or composite or distorted in character, or form, or based on or involving any stunt poses, in conjunction with my own or a fictitious name, or reproductions thereof in color or otherwise, or in derivative works made through any medium, where said depictions may include any or all of still, single, multiple or moving photographic images or pictures.

For valuable consideration, the Model hereby grants the Photographer the irrevocable right and permission in connection with the photographs taken of me, or in which I may be included with others, the right to use and reuse, in any manner at all, said photographs, in whole or in part, modified or altered, either by themselves or in conjunction with other photographs, in any medium or form of distribution, and for any purposes whatsoever, including, without limitation, all promotional and advertising uses, and other trade purposes, as well as using my name in connection therewith, if he so desires; and the right to copyright said photographs in his own name or in any other name that he may select. I waive the right to inspect or approve any use thereof.

Model does hereby waive any right to inspect or approve the finished product or advertising or other copy that may be used in connection therewith or the use to which they may be applied. Model hereby forever releases and agrees to hold harmless and forever release and discharge from any and all claims, actions and demands, the Photographer, his legal representatives, assigns, and all persons acting under his permission or authority, from any liability involving studio or set locations injuries or accidents, picture image blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of the pictures, or in any processing tending toward the completion of the finished product, unless it can be clearly shown that the publication thereof were maliciously caused, and produced, and published solely for the purpose of subjecting Model to conspicuous ridicule, scandal, reproach, scorn, and indignity.

Model hereby affirms that all poses, positions and situations enacted in the Depictions covered in this release were entered into without force, coercion, or threat whatsoever, and were posed freely by Model with Model's full consent in return for compensation and for Model's own amusement. Model further agrees to hold blameless and free of all accusation of such force or coercion Photographer, his legal representatives, assigns, and those acting under his permission and on his authority. Venue/jurisdiction of any and all dispute(s) shall lie exclusively in the District Court of the State of Washington for the County of King.

_______ (initial) Model hereby affirms that he/she is over the age of eighteen years and is fully able to contract in his/her own name without breach of any prior agreement or applicable law. (if a minor under the age of 18, do not initial and the parent or legal guardian must also sign this release).

Model's compensation in connection with this instrument is:

(a) The Photographer will provide file copies of final images to the Model in the form of a CD, DVD, or via download over the Internet (does not apply to film images). The Photographer shall charge no fee for this. Prints will be provided as follows: 8x10 or 9x12:1 per hour, or, 5x7/4x6:2 per hour, of total session time. Files and prints will be provided within a reasonable time (ex: 2-4 weeks) following the session. Failure of Model to respond to communications after the shoot regarding proofs, prints, or delivery within 2 weeks of notification will nullify and forever cancel the Photographer's compensation requirements and liabilities herein.

(b) The Photographer does hereby grant the Model reproduction rights on all copyrighted images. The Model may use these for their own self-promotion, including publishing and distributing (hardcopy and/or via Internet) said images as long as the Photographer’s copyright © notice remains intact, or is otherwise attached or stated with the image.
